Welcome to Concept3 LLC
All new and potential clients should read and complete the following documents, bringing them to their first session. The CLIENT INSURANCE FORM 2014.pdf must be returned a minimum of seventy-two(72) hours prior to your intake appointment, via email or fax, as most insurance companies do require prior authorization. Please make sure you include your contact phone number so that I may call you prior to your appointment to confirm that we have the appropriate authorization codes.
Please complete the remaining paperwork and bring it with you to your first appointment. Feel free to make an extra copy to retain for your personal records.
Two days a week I employ Animal Assisted Pet Therapy with a small Belgian-Malenoise, Cameo (“Cami”) and a Pyreness, Shebah Bear. Clients who do not wish to participate in Pet Therapy, may schedule on a day when the dogs are not present. The office was designed with “allergy sensitivity” featuring: laminate flooring, leather furniture, experiential therapy containers, and minimal carpeting, which is cleaned nightly. Please advise therapist - if you or your family member needs consideration for their allergy needs.
